Moving out of the city and leaving Japan
Please submit a notification of moving out when leaving the country or moving to another town or city.
If you do not have a My Number Card and move to another town or city, please obtain a Change of Address Certificate from the City Office. This Change of Address Certificate is required when you register as a resident in the new town or city.

Individual Number (My Number) Card
A My Number Card can be used as an ID card and a health insurance card. Application is optional, and you can get one for free the first time. By obtaining a My Number Card, you can issue various certificates yourself at convenience stores.
